Are the heroes supposed to start on the entrance tile when a location isn't specified or do they start off map and enter via the entrance squares?

+1 same question. I think heroes start off map and enter via the entrance squares while the Overlord start with his monsters on the map… but I'm note sure.

If you look at the setup diagram on page 6 of the rulebook, you'll see that the heroes start on the map.
